$500 penalty payment for each violation
Fill in the form with required information (in yellow*). Submit the form to the offending party and the Utah Attorney General at 350 N State St Suite #230, Salt Lake City, UT 84114.
*Note: "Part 1 Description" is an example. You must research which books are available in your school. Check the online database (SORA etc) and your physical library databases. Document your findings with pictures and screenshots. Note the locations and dates.
Physical libraries have a different requirement with a date of 5-12-2020, so we have compiled a list of books published after 5-12-2020. See the second tab of this spreadsheet for that list.
Purchase Stickers
Offending parties may purchase the stickers below to satisfy the law for physical copies.
